To register for CCD:
1) Fill out the CCD/Youth Ministry Registration form below (one per family).
2) Fill out the Emergency Medical below (one per child).
3) Sign up for Flocknote here. Add your name to each grade level your children are in.

One form for each child participating in ANY St. Remy event or group. Please fill out a new form by July 1 each year. They are valid July 1-June 30. |

Bible Camp forms due July 24 (to get a t-shirt): |
Brotherhood Rugby permission slip for July 11, 25 & Aug 1 You must also submit an updated Emergency Medical form (see above). |
